	Application and Other Forms: Application forms are available during the tour, and on-line. ASP Application is also available. ASP Full Day Form is due monthly to reserve your child's spot. The Health Appraisal Form (due within one month of admission and updated yearly) and the Non-Prescription Medication Form (required for all over-the-counter medications) are available on-line. 	Fees & Deposit: Application requires a non-refundable fee of $25. Tuition fees are due on the first of each month, and can be found on the Application form. When a family applies to EEC Inc. and an opening is available, you will receive a Confirming Enrollment/ Fee Agreement letter requesting a deposit of $300 (for the After-School Program (ASP), the deposit is $100).
