	Application and Other Forms: Application forms are available during the tour, and on-line. The Health Appraisal Form (due within one month of admission and updated yearly) and the Non-Prescription Medication Form (required for all over-the-counter medications) are available on-line. (You'll need the Adobe Acrobat Reader to download the forms. Click the link to download.)
	Fees & Deposit: Application requires a non-refundable fee of $25. Tuition fees are due on the first of each month, and can be found on the Application form. When a family applies to EEC Inc. and an opening is available, you will receive a Confirming Enrollment/ Fee Agreement letter requesting a deposit of $300 (for the After-School Program (ASP), the deposit is $100).
